Using the Pharmacy Mail Order Enrollment Form Upload tool is easy.
- Fill out your order form.
- Select the 'Choose File' or 'Browse' button to browse and locate your file to upload.
- Click the 'Upload File' button once you have selected your file.
- Wait for the Success or Fail message to appear.
